Authorization Agreement for Electronic Funds Transfer (EFT)

I (we) hereby authorize City Center Parking, hereinafter referred to as "Company", to initiate debit entries to my (our) account indicated and the depository named below, hereinafter referred to as "Depository", to debit the same to such account for the monthly amount of validation charges applicable to customer’s account. This authority is to remain in full force and effect until Company and Depository have received written notification from me (or either of us) of termination, at least 5 days in advance of such termination.

In addition, you must provide us with a voided check in order to complete your EFT application.
