Validations EFT Application

Please Note: This form is for Validations accounts only.  If you are a Monthly Parking customer, please click here to be redirected to the Monthly Parking page.  Then, locate your lot and click the link under 'Payment Options'.

Instructions:

  1. Enter the information requested on this form. Fields with an Asterisk(*) are required.
  2. Read the Authorization Agreement for Electronic Funds Transfer (EFT) and enter an initial amount.
  3. Provide us with a voided check.
  4. Click the 'Submit' button on the bottom of this page when completed.

Authorization Agreement for Electronic Funds Transfer (EFT)

I (we) hereby authorize City Center Parking, hereinafter referred to as "Company", to initiate debit entries to my (our) account indicated and the depository named below, hereinafter referred to as "Depository", to debit the same to such account for the monthly amount of validation charges applicable to customer’s account. This authority is to remain in full force and effect until Company and Depository have received written notification from me (or either of us) of termination, at least 5 days in advance of such termination.

I have read the Authorization Agreement for Electronic Funds Transfer (EFT).

In addition, you must provide us with a voided check in order to complete your EFT application. You can email or fax an image of the voided check or you can mail or deliver the voided check to our office. Please indicate that you have completed the on-line EFT Application when you submit your voided check.
